Wabash Community Health Center is a non-profit organization dedicated to providing quality, affordable health care to all members of our community - which means we treat patients regardless of their ability to pay for services.
Our health center provides primary care and behavioral health services and is located in Mt. Carmel, Illinois at 1123 Chestnut Street. As of April 1, 2021, the Wabash Community Health Center received Federally Qualified Health Center (FQHC) look-alike status by Health Resources & Services Administration (HRSA). Federally Qualified Health Centers are community-based health care providers that receive funds from the HRSA Health Center Program to provider primary care services in underserved areas.
We have three providers available to serve your health care needs. Our primary care provider, Casey Ikemire, APRN, FNP-C, MSN, FNP-BC, sees patients of all ages. In addition to addressing physical issues at our clinic, we have two psychiatric nurse practitioners, Logan Keepes and Leann Lear, to work with patients to ensure that their emotional and behavioral issues are treated.
This practice serves all patients regardless of inability to pay.
Discounts for essential services are offered based on family size and income.
